Virginia's Emergency Department Care Coordination (EDCC) Program, launched in 2017, is transforming the way healthcare providers collaborate across the state. By connecting care teams and delivering real-time data, this innovative program enhances coordination, improves patient outcomes, and streamlines the care process.
Through the EDCC, hospitals across Virginia securely share encounter data, enabling seamless connections with health plans and healthcare providers. Participants have access to vital information, including:

- Patient demographics, including contact information and insurance details
- Insights into personalized care plans and patient care team members
With over 400 participating facilities and more than 6,000 healthcare professionals already on board, the EDCC program is creating a unified network for enhanced collaboration and better patient care.
